These terms govern use of the fisiobox.co website and of the online appointment-request service.
Acceptance
Using the site and submitting the appointment request form means you accept these terms and the privacy policy.
Appointment request service
The form lets you request an appointment; it is not a confirmed booking. A member of the team will contact you, normally within 48 hours, to confirm a date and time.
You must provide accurate, current information. Clinical information you supply is used only to prepare your assessment.
You must be of legal age to submit the form. If the request concerns a minor, it must be made by their parent or legal guardian.
Cancellations and missed appointments
Cancellation policy: MINIMUM NOTICE PERIOD, e.g. 24 hours
No-show policy and any applicable charge: STATE WHETHER A CHARGE APPLIES
Prices and payment
STATE WHETHER PRICES ARE PUBLISHED ON THE SITE, ACCEPTED PAYMENT METHODS AND INVOICING POLICY
